飛航安全自願報告系統係為主動發掘潛伏性飛安危害因子，藉由蒐集各類可能危害飛航安全之資料，經由調查與統計分析，採取必要作為，防患事件於未然。

依據個人資料保護法第15條第1項第2款：「公務機關對個人資料之蒐集或處理，除第六條第一項所規定資料外，應有特定目的，並符合下列情形之一者：二、經當事人同意。」，為發掘潛伏性飛安風險因子，需提供您的個人資料，本報告系統為非懲罰性之保密報告系統，您的個人資料將於處理後移除。

針對您所報告的內容，本局如有必要取得更多資訊，或您於報告內容中勾選需要回覆等情況下，本局將主動與您聯繫。

The establishment of the Flight Safety Voluntary Report System is intended to identify potential hazards to flight safety. Through the collection, investigation and analysis of various information relating to flight safety risks, necessary measures are taken with a view to preventing the occurrence of flight safety incidents.

Under §15.1.2 of the Personal Information Protection Act “…the government agency should not collect or process personal information unless there is a specific purpose and should comply with one of the following conditions:… 2. consent has been given by the Party”. For the purpose of identifying potential hazards to flight safety, your personal information will be required. The Aviation Safety Voluntary Reporting System is a non-punitive, confidential reporting system. Your personal information will be removed after it is processed.

You will be contacted if we find it necessary to obtain further information with respect to your report, or when you tick the box in your report that the reply is required.
